redesign the IIFYM Diet Calculator page for better conversions

IIFYM.com is a wordpress diet website has been free to users since we started (4 years ago).
We are converting the website in to a free membership site, with upgrade options users can pay for.
Most of our traffic goes to our IIFYM calculatore, located here:

previously, users could use this feature for free. Now, we require that register/log in to get the results our calculator provides.

We need this to have a modern, yet VERY SIMPLE to use look and feel.

I need our page redone to look very modern and easy to use.
All of our current functionality needs to be represented but the layout and design need a massive overhaul.

Some space for SEO content is required.
