Simple, clean design for new web startup

IntroductionAgent

IntroductionAgent is a simple web application to facilitate double opt-in introductions. We're a brand new web startup, so our budget for this initial project is very small. However we plan to hire the winner as our long-term designer, so you can make much more money in the future.

Business people, entrepreneurs, venture capitalists, bloggers, etc.

See the attached PDF file. It shows the wireframes for five screens in the application.

IMPORTANT: You're only required to design ONE page! Please design the third page, called "IA_home".

If you want to submit designers for the other screens as well after the home page is approved, we would appreciate it and it may increase your chances of winning (if we like your overall design the best, that is). However this is up to you and not required.

You are to create a usable, clean, simple web UI. The design should not have any extraneous design elements, but should be as simple and clean as possible.

Note: You don't need to design a logo. We won't have a logo initially. Just put the name "IntroductionAgent" in a font you like.
